I ______________________________hereby give my permission to my son/daughter _____________________________ to participate in their Major Field Trip and Seminar as part of their requirement in partial fulfillment for the degree in Bachelor of Science in Computer Science/Bachelor of Science in Information Technology. This trip will be held from October 4 to 8, 2010 at Manila and some areas in Luzon.
I fully understand that my son/daughter is to abide by all rules and regulations governing conduct during the field trip and seminar. I shall by law, be deemed to have given up all claims against the University of Southeastern Philippines and each of its officers, faculties and staff for any injury, accident or illness occurring during or by reason of the field trip. I also agree to relieve the district of any responsibility for damage to or loss of my son/daughter’s property occurring during the field trip and seminar.
